Pharmacy benefits for U.S.-based employees are offered through CVS Caremark, a pharmacy chain and pharmacy benefit manager that handles about one billion prescriptions per year. CVS Caremark provides our company with significant purchasing power, saving you and the company money, and offering you the flexibility to select the most convenient way to receive medication – either via mail, at any local CVS/pharmacy®, or at the on-site pharmacy in Cedar Rapids. There are more than 7,000 CVS/pharmacy locations across the United States available for you. A store locator is accessible on the CVS/pharmacy Web site.
Register online to get the most from your prescription benefit
Register at CVS Caremark to actively manage your prescription benefit. The site contains information about mail orders or how to place them, as well as Rockwell Collins-specific information and prescription history. You also can explore ways to save money on your medications, manage your prescription benefits, access valuable health and wellness topics, and more.
Be sure to have your Prescription Card with you to complete the registration process.
You may choose to receive your medications through the mail. Mail orders are available for 90-day supply* prescriptions of either maintenance drugs or non-maintenance drugs. For more information about mail orders or how to place them, visit CVS Caremark.
*Actual quantity may vary depending on your plan.
Please allow 7-10 days for mail delivery from the time your order is placed. If you need your prescription right away, ask your doctor or other prescriber to write two prescriptions. The first for a short-term supply (up to 30 days) to be filled right away at a participating retail pharmacy. The second prescription would cover up to a 90-day supply, with as many as three refills (if appropriate) to be filled through CVS Caremark Mail Service Pharmacy.
New prescriptions by mail
If you need a new prescription, there are several simple options. Your doctor’s office may place the order for you using the FastStart® physician number, 1-800-378-5697. You will need to provide your doctor’s office with the identification number from your CVS Caremark Prescription Card and your mailing address.
You also have the option of ordering online, by phone or mail. Online and phone orders also can be placed through FastStart. Access the program online at www.caremark.com and click on "Start a New Prescription." Once there, click on "Order a new prescription now through FastStart." Access FastStart by phone via the toll-free number 1-800-875-0867 and CVS Caremark will contact your doctor to verify a new prescription.
Lastly, you may order a new prescription by mail. Send the prescription and a completed CVS Caremark mail service order form to:
PO Box 659541
San Antonio, TX 78265-9541
Be sure to include your original prescription; photocopies are not accepted.
Mail order prescription refills
All refill requests should be submitted to CVS Caremark. You have the choice of three options to request a refill:
- Online. Register at CVS Caremark to order refills and check the status of your order any time of the day or night.
- By phone. Call Customer Care toll-free at 1-800-935-5834 for fully automated refill service. Have your ID number, found on the front of your Prescription Card, ready.
- By mail. Complete the CVS Caremark Mail Service Order Form and mail it, with the appropriate copayment, to CVS Caremark. If you have not ordered prescriptions from CVS Caremark, write your prescription numbers in the space provided near the bottom of the front page of the mail service order form.
With each order you receive, you also will receive an order form that has your prescriptions listed. To order prescriptions using this personalized refill form, simply fill in the ovals for the prescriptions you want to refill. If you need to refill a medicine that is not listed, write the prescription number(s) in the space provided.
